"Work" in an organization is seldom designed, rather it “evolves” to keep pace with gaps in performance exposed by growth and change.  Small problems are often ignored in order to focus on critical performance gaps.  Under pressure for time and results, “interim solutions” are used to close these critical gaps, teams are rewarded for their heroic activity, and the cycle repeats as the smaller problems grow and must be addressed.  The best time for the more robust solution never occurs so the interim solutions approach becomes normal.

The "Cycle of Heroic Action" is a learned behavior that over time creates complicated work processes to complete even the most simple tasks.  Work process often resemble a "Rube Goldberg" machine.  Functional, fun to watch, but very confusing to customers, inefficient, difficult to change or scale, and prone to breaking

Learning how to make work simple and close gaps when they are small, is the key to breaking the cycle.    Companies that can shift their thinking and demonstrate a collaborative capability to recognize and surface problems, adapt, and respond quickly with effective customer valued solutions  will win and grow.
